For example I missed one about investigating a military death. I found it, went back to my ship had 1-2 cut-scenes but no travel then went back to the planet and the timer had already timed out. Even though there was no warning.

为了避免各位掰盘砸显示器的悲剧，必须转这个警告：

There are a series of actions that must be performed in order for the Geth and Quarians to coexist. In order for the best scenario you need to have 5-7 points determined by these factors:
Destroyed the Geth Collective in ME2: 2 pts
Tali not exiled in ME2: 2 pts
Paragon/Renegade action used to resolve conflict between Tali and Legion in ME2: 1pt
Geth server mission completed in ME3: 1 pt
Rescue Admiral Koris in ME3: 1 pt

ME里头好多外星种族都只有一个性别，Bioware你究竟是有多懒……果然有蓝皮就够了么……
salarian没女的，好像有一种说法是雌性只呆在母星不出去
水母人大象人鼹鼠人什么的都没有
倒是四眼人(Batarian?)有女的……虫子(Rachini?)也算有吧……

不过第一个那个是指的只要完成LEGION的忠诚任务就算数还是得选毁掉异端（而不是改写）才能拿到这两点
